Your first expedition starts here

Before your first visit

We use secure online forms so you can complete everything ahead of time. Parents tell us this is one of their favorite things about our office: paperwork done from the couch, and a wait of minutes, not an hour.

What to bring

• Your photo ID and your child's insurance card
• A list of any medications your child takes
• Completed online forms (or arrive 15 minutes early)

Good to know

• We treat children and teens up to age 18
• One guardian accompanies each family in the treatment area
• Georgia Medicaid, Peachstate, Caresource, Amerigroup and most PPOs accepted
